Three edits to the original essay, made in response to catches in the comments.
1. Aspirin dose error (caught by Cprodge).
The original paragraph placed the 81 mg "cardioprotective" tablet within the 1 to 2 g/day range. It is not. 81 mg is 0.081 g, well below that range. The actual literature has three tiers, not two: above 3 g/day, aspirin is uricosuric (increases urate excretion); at 1 to 2 g/day, it causes urate retention; at 75 to 325 mg/day, which includes the 81 mg cardioprotective tablet taken by tens of millions of Americans, it also causes urate retention via the mechanism Caspi and colleagues documented in 2000. The paragraph now distinguishes the three tiers correctly, and the Caspi finding is placed in the dose range Caspi actually studied.
2. Alcohol added as parallel substrate to refined fructose (caught by Greg Pilcher, with pogi, Ray Horvath, and David Currie surfacing the same point in different framings).
The original essay omitted alcohol as a documented cause of gout despite Hyon Choi's flagship 2004 Lancet cohort study on the topic. A new paragraph in Cause One: The Industrial Sweetener Pipeline now covers the parallel mechanism. Ethanol metabolism in the liver generates lactic acid that competes with uric acid for the same proximal tubule transporters the fructose-induced lactic acidosis crowds. As always, nature supplies a solution: Goutweed (Ground Elder, or Bishop's Weed). This invasive plant was traditionally used externally, as a poultice of crushed leaves applied to the inflamed joint. It is quite edible, though, and has anti-inflammatory, anti-bacterial, and anti-oxidant qualities. The rhizomes even contain a unique plant lectin, of a higher molecular weight than all other plant lectins. (This research was published in 1985, )This makes me think that the research on the potential medicinal applications of this plant still remains to be done.
The essay mentions three causes of gout -- industrial sweeteners, lead, and pharmaceuticals. I'm surprised not to see alcohol mentioned. (The essay does mention some long ago alcohol preparations that involved lead-lined vats and vessels, but it does not suggest alcohol consumption itself as a cause of gout.) Alcohol reportedly causes gout through a process similar to that mentioned in the essay; namely, that metabolizing alcohol generates uric acid from the breaking down of purines while simultaneously producing lactic acid that competes with uric acid for elimination by the kidneys. Unbekoming -- do you intend to discount alcohol as a cause of gout?
I personally had gout from my beer addiction, although I was also eating animal proteins and too much sugar, so it was likely the combination of things? I quit drinking alcohol (again), and am trying to eat healthier. I'm 70 and have a metabolic disorder or two and liver problems - hepatitis C supposedly, but I don't believe in viruses. I enjoy this substack, and others which are related to the weaponized 'medical' system (allopathic fascism?) being used against us. I assume the reason alcohol wasn't mentioned was more of an oversight than not believing it contributes to gout. Those injections most people get probably contribute too?
About 6 months ago we had a cat develop blood in his urine, hiding and obviously not well. 6 months before that his friend had a urethral blockage and had to be euthanized. I was told the condition was idiopathic. I suppressed my laughter even in the tragedy of the moment and thought Yup. It's a pathology of idiots. I didn't say it as I needed the ER vet to like me at that point. Since then I have discovered that indoor living and kibble are killing our pets. Mars [ yes the candy bar ] is buying up all the veterinarian practices and what cost you 100 dollars in 2015 now cost 4000. Surprisingly - in a moment of transparency the ER vet said that indoor cats are their bread and butter because they are under so much stress being unable to go outside and be cats, that the chronic illnesses are exploding. I mean seriously, cats with IBD have a Facebook page??? What they do to us, they are now doing to our animals and I find it peculiarly heinous that they are exploiting our love of them for money. Indoor cats don't get grounded/earthed from being in contact with the ground, which leads to many health problems. Service humans who get grounding mats for their kitty can't keep them off of it, and their health problems usually disappear. It's been proven that grounding increases negative ions, reduces systemic inflammation, lowers cortisol, and has numerous healing effects. Kibble IS horrible to feed them, thankfully pet foods are getting better, but you can make much healthier food for cats and dogs and save money by making it yourself using human grade food. There are many websites, videos, and books explaining how to do it, but it's self-evident. I'll end my comments, since this is digressing away from the subject at hand.
We in Europe have lived half of our life with leaded gasoline filling the ambient air in town and along roads with heavy traffic. It has been stopped at the end of the eighties, but before all gasoline was leaded...
Today federal authorities measure relatively high lead-contents in the upper soil adjacent to roads on a breath of several dozen meters on both sides. There is surely an impact of that pollution with consequences for our health? Thank you for your initiative!
Still this leaves me wondering about gout in other historical periods like the Renaissance. Montaigne mentions it in at least one essay and apparently it was well known to doctors as a disease of the upper classes. I suspect they did not use lead pipes. They would have eaten a 'whole food diet.'
Lead water piping was widely used in the UK until modern times. It first began to be removed and replaced (usually by copper) in areas with soft water. On average, there are more soft water areas in the north and west.
In hard water regions, a layer of scale builds up on the pipe and reduces the amount of lead which is dissolved and enters the water. In hard water areas, I suspect that most but not quite all of the lead piping was removed. Hard water is more common in the east and south-east.
Also, parts of Derbyshire and other counties used to be full of lead & zinc mines. Beware of the tap water too, even if it comes through plastic and copper pipes?
I looked at the Wikipedia article and apparently the USA removed lead from solder in the early 1980's. The EU, naturally, has more and more detailed regulations. Plastic plumbing with a failure mode (PolyB) was introduced in the late 1980's, so our domestic water in residences built earlier has a good chance of flowing through lead-soldered copper plumbing.
The last time I checked lead levels in clinic was when I did miners' medicals for Cominco in the early 1980's. I had not seen an association with gout until this article, but I had wondered about the Royal Navy's admirals for some time.

Obviously, environment, diet, etc., all play a role in every health issue. But, as a periodic gout sufferer, I can tell you one thing that really works: regular blood donations. It has to do with oxidized blood, and excess ferratin build-up.
Consider that women rarelly if ever have gout attacks until they are much older. They have a cycle and are always replacing blood.
I have maintained a healthy weight all of my life and don't partake in sugary drinks or much sweets. I will add one thing: If you DO get a gout attack, you will be in so much pain that you won't care what is good or bad for it. You'll want relief and the little indomethacin pill will become your friend whether it's good for your long term health or not.
I've donated blood regularly for 7 straight times and have not had an attack in over a year. Thank God.
very interesting points. Recently had a detailed CBC / and other blood tests and they found high ferritin levels. I have not had a gout attack in years but this correlates with having one. I may consider giving blood now.

I can think of several people who complain of gout... and are known to be very fond of soft drinks, sugary desserts, and excess caloric intake. Bill Gates and Warren Buffet are, as I am told, huge producers of high fructose corn syrup. They are being enriched by the high volume of consumption of high fructose corn syrup. While in the Yucatan, I noted that even canned tomatoes were somehow deemed to be in need of high fructose corn syrup. Personally, I despise the overuse of unnecessary ingredients, like high fructose corn syrup in foods where there seems no good reason to have it there. Yet, the usage extends even to other foods, even meats, mysteriously, and I can only surmise an ulterior motive exists for putting the sweetener therein... to cause addiction to the product, repeat purchases, and increased profits, from folks who know noting about nutrition and disease causing food additives. Thank you for this article, it will be used to help others.
Maybe the fourth cause of gout would be vaccine? Esp early childhood vacs like MMR ? I have seen my blood under dark field microscope and i can see certain sharp crystals are swimminh with red blood cells,,, “uric acid crystals”? Later in my life i change diet to a better one (without hfc, seed oil, etc) but gout attacked still happened esp when i was dehidrated. i tried sodium bicarb and has very good result within 1 hour reduce the pain tremendously (of course paired with lots of drinking water)… Later on when i got anesthetic injection (for teeth removal), the symptom is returned even with greater pain. Sodium bicarb doesnt seem to work any longer. I switch to noni fruit juice to cleansed the blood. It takes a week to have my pain reduced. Later on I tried chlorine dioxide which works better, but the preparation is complex. I replaced it with H2O2 5% 10-15 drops in 500cc and works great. I still have to be dilligent with my water intake (plus sea salt), otherwise my feet start hurt again.
Yes. The 2 toe gout episodes I had were in the height of hot summer weather when I was physically working and got dehydrated. Doc recommended a NSAID (which I had never taken before) and in 24 hours my stomach exploded in burning pain. Went to chemist and got some stomach-protecting drug that they said fixed what is a known NSAID side effect. What a shit-show! I take no Pharma drugs, live remotely on a farm and eat locally-grown food.
Looking at the list of possible causes here, none of them apply. I haven’t had any problem since the 2014 and 2017 episodes but the matching terrain in those incidents was 1) working in above 100 degree heat 2) Not staying hydrated as it was inconvenient to go to the toilet very often 3) stressed to the max with the projects I was working on.

Yep, I did not have near the problem you did but I completely solved my gout with simply drinking more water every chance I got. . . Get up mid of night, drink some water, Standing around near a sink, drink some water. . .
I never kept track of how much per day but not as much as many folks who go everywhere with their goofy water bottles.
I was on allopurinal for about a year and became allergic(hives), so had to stop cold turkey. Was re-educated about drinking more water and less booze / beer and changing those two simples things fixed it. I still have a beer or two a week.

Please keep track of your kidney filtration rate. My dads rate got worse as he aged. For him, he slowed down meat at 76 and stopped at 79, and his kidney filtration rate improved.

Beer is the most inflammatory of the alcohol types and high in purines. Sugar and beer are big drivers of elevated UA.

They did use crystal decanters and glassware. There was a movement in the early 1990's to remove the lead from crystal glass.

My husband has battled gout for 12 years. We have tried medication and diet but nothing has helped. This article brings up some interesting points.

Would you consider writting about how allulose helps alliviate gout?
in my comment, i make suggestions that will stop the sources of uric acid production.

the solution would be to stop eating meat, fats/oils, fake chemicals (HFCS), lead, and poisonous chemicals pretending to be medicine (aka pharmaceuticals)
Did all that. My most helpful item was allulose.

bare in mind, that animal protein has its own challenges. Carnivore live shorter lives than herbivores for that reason.
Please keep track of your kidney filtration rate. My dads rate got worse as he aged. For him, he slowed down meat at 76 and stopped at 79, and his kidney filtration rate improved.
