It would be terrifically helpful if you could tell us how we could find and reasonably use these bio-identical hormones without a good doctor or lab, because these are very expensive and hard to find. Also, you’ve mentioned many times the importance of thyroid function; it would be good to know some helpful things we can do to improve that on our own. Also, what about people who have had their thyroid removed, as so many have?

How to find reputable and knowledgeable doctors who specialize in hormone optimization and evaluate all hormones mentioned in this article and not just testosterone and estrogen?
I'm not convinced that everyone needs hormone therapy as they age, but a relative of mine just started the typical perimenopause hormones. I had already heard about bio-identicals being much better, but she says they are so much more expensive and can't afford them. I suggested that the first thing would be to get her Vitamin D level in a better range, change the diet a bit, and lift weights, but are there any affordable options for bio-identicals?

I have zero disposable dollars for expensive (or even inexpensive!) hormone therapy however will up my vitamin D intake and buy some melatonin.
